Q1: Clever. Simple version of my ancient idea of cycling questions, but your implementation seems simpler, I think. Might be a bit weird with timestamps, but those are edge cases.
Rest look good.
Suggestion: Add a note that it might take time for acceptance, and that it is sometimes in bursts.
Other question idea:
Name an admin besides you.

Im hoping that we don't just get rid of griefers, but we get better, more active players. You do seem to have to put a lot of time into your app and some lesser players might just not bother. Question 1 does this very well. If you wanted to go further, you could ask for build screenshots or a long description of building skills.

Wow. Great job on the changes. That will really sort out griefers I think.
If I were a griefer I wouldn't take the time to fill that out

It most certainly will prevent griefers. Plus one awesomeness! However, it may deter others due to the complications of it all. I probably wouldn't be here if this was in effect when I came around. But that's just me, so no loss for others. And no awesomeness either.
(I'm on here far more often now. Weird, eh?)

Yer because griefers tend to be people who are bored of minecraft, they have played the game and have realised that they cannot build anything decent. So they mess around with mods and then that becomes boring they go on pvp which then in itself becomes boring. They then decied to just hceck on random server and grief.
